Hart-Supported Living Learning Outcome

Hart Supported Living is a program of the Department for Aging and Independent Living that allows Kentuckians with a disability the option to create their own individually designed plan of supports. This program provides ongoing and one-time request funds, including funds for home modifications, service animals, caregivers, and more. As a result of the completion of this module, you will understand Hart-Supported Living and the types of funds this program provides.

Welcome to Hart-Supported Living, a training created by the Transportation Initiative.

EPSDT

Early and Periodic Screening, Diagnosis, and Treatment

Early and Periodic Screening, Diagnosis, and Treatment – or EPSDT – is a program administered by the Department of Medicaid Services. EPSDT provides regular health screening services and medically necessary special services to children and young adults who qualify through Medicaid or KCHIP enrollment. Transportation may be provided to these appointments and services. As a result of the completion of this module, you will understand EPSDT transportation options.

First Steps

First Steps

First Steps is Kentucky’s state-wide early intervention system administered by the Department for Public Health in the Cabinet for Health and Family Services. First Steps provides services to children with developmental disabilities from birth to age three. Services may be provided at home, in center-based programs, or in clinical settings. As a result of the completion of this module, you will gain an overview of First Steps and learn how to access transportation supports within the program.

Community Integration Supplement

The Department for Aging and Independent Living (DAIL) and the Kentucky Transportation Cabinet Community Integration Supplement, or CIS, provides money to help those with a serious mental illness to live in the community. These funds can be used to pay for transportation needs. The Department for Aging and Independent Living (DAIL) and the Kentucky Transportation Cabinet Office of Transportation Delivery (KYTC-OTD) have formed a partnership to provide accessible transportation to COVID-19 testing sites for individuals with disabilities. As a result of the completion of this module, you will understand the transportation supports available in Kentucky to get to a COVID-19 testing site.

Kentucky Assistive Technology Loan Corporation Learning Outcome

The Kentucky Assistive Technology Loan Corporation (KATLC) provides loans for equipment that will improve the quality of life or increase the independence of Kentuckians with disabilities. This program has provided more than $7 million in loans to Kentuckians with disabilities to obtain the assistive technology they need. As a result of the completion of this module, you will understand the transportation supports available through a loan from KATLC.

Blind Work Expense Learning Outcome

Blind Work Expense (BWE) is a work incentive program of the Social Security Administration. By deducting a wide variety of eligible expenses, people with a diagnosis of blindness can recover both work and disability-related expenses and retain some or all of their SSI check.  As a result of the completion of this module, you will understand Blind Work Expense eligibility, qualifying expenses, and the impact on SSI and income.

K-TAP Kentucky WORKS Program Learning Outcome

The K-TAP Kentucky WORKS Program is a service available to Kentuckians who are unemployed, under-employed, or about to become unemployed and are pregnant or responsible for a child under 19 years of age. This program offers monthly transportation support payments and yearly auto repair funds to work-eligible individuals. As a result of the completion of this module, you will understand the transportation supports available to K-TAP eligible households through the Kentucky WORKS Program.
